If it happens on one bike, check the bike first, if it happens on more then one bike, check whatever is the common factors. If it's not the shoes or the cleats, I would look to see if the cleats might be worn, there could be some play in the cleat, that is allowing it to move under certain conditions. I would take the shoes to your LBS and have them look at the cleats. If it's not the shoes or the cleats, then the only other thing that is a common factor is the engine.
