What do you mean by a "gash"? Is the bearing ring split or is the track where the balls run pitted or deeply worn to a ditch like shape? Any of these means you need a new headset.

Not sure what you mean about your threads being "crushed" either. That would imply that they were cross threaded. But sometimes steer tube threads are not the best looking threads either. The true check is how well the headset threads on and off the steer tube. If it's rough and stiff to turn on the threads and if it seems tilted to the tube then it's cross threaded. And that means you would be best off to look at a new fork. And if the headset is messed up anyway by a split bearing race or damaged track on a race then this would be a good time to go with a threadless replacement setup.
