My LBS cut my girlfriend's with a hacksaw. I think it all costed around 60 dollars, they charged me $1 each for five 5mm spacers. Don't know if that's normal.
At my LBS $60 got me a cut and threaded chromo steerer (NOS mavic carbon fork & chromo steerer), and a custom new crown race to boot... installed....
