In my experience, the main reason for elbows hurting is the fact that you've got them locked through most of the ride. You need to keep at least a small amount of bend in your elbows - it absorbs shock better, it helps you relax in controlling the bars (no death grips allowed), and it helps keep you at least a teeny bit more aero. It also emphasizes the need for core conditioning.
