Sounds a bit like chilbains. Keep them warm but, counter intuitively, don't warm them up rapidly when they are cold. My wife suffers from these, probably as a result of long-undiagnosed celiac's disease. Bare Feet has the right idea: see a physician (or two or three, until you find one who can help).
