I think death might be a little overstatement of the risk. (Although, when you are in the middle of failing a clip-out you may wish you were dead.) The risk vs. reward ratio is way in their favor. I think we all are WAY more in danger of being killed by simply riding in the proximity of cars than we are being clipped into pedals.

My only fail was shortly after converting over. I was finishing my ride and coming up to my van to load the bike up. I was conscious that I needed to unclip--just waited it a bit too long. . .It was all in slow motion. . .As I gathered my wits about me a uber skinny full-kit roadie came rolling through on some CF wunderbike and looked back to ask if I was okay. In my mind I wanted to tell him to ride away and leave me in my shame. It came out of my mouth as a meek/dejected "Yeah, I'm fine. . ."
