I saw this happen to a buddy of mine once...shoes and cleats were fairly new....but something happened and one just ripped out of the shoe....lbs said they never saw that before....he was lucky and didnt go down...thye replaced it and he never had another issue.
I have had 2 pairs of SIDIs and only issues I have had is occasionally losing a screw, and stripping the aluminum extender plates once...
Always check the screws as part of pre ride bike check and use loctite.....
Your so right and lessened learned.For now on i will tighten those screws .Im really good about taking care of my equipment but the shoes never occurred to me.thanks
