I'm a true US 15 on a Brannock and find 51 to generally be just a little tiny bit big and 50 to be a little too snug, so wear 51's.
I am a 15 in every shoe I buy and I wear a 50 in sidi. I have both their road and mtb shoe and they fit fine. Shimano says 51 equals a 15 us...haven't tried on on however. I found mine on amazon and zappos.
