Can't tell for sure from the photo, but here goes.

1- the ball cages go on the two conical parts in the same orientation they're in now.
2- Then the plain cone (2nd from top) goes on the fork, press it on using a piece of 1" water pipe as a slide hammer.
3- the lower cone and it's ball cage is inverted and threads onto the fork.
4- the keyed washer is next, and the locknut is last.

That's the basics, search the net for a tutorial on headset assembly and adjustment. Don't forget the grease.
