I'm a solid 11.5 D dress shoe. Sidi 46 std. width Genius 5 or 6.6 fit me like a glove after a tiny bit of break-in.

Because it is. You don't need to cushion for impact in a cycling shoe. Just enough to conform the last to your foot without allowing movement. The tension straps should provide enough volume adjustment for heavier socks.
