I concur, looks like he let himself get hooked. My elbows come out, too; because I'm short, my handlebars are a lot lower than a lot of other guys'. It's easier to get your levers hooked if your bars are lower.

But it's pretty hard to tell for certain what happened based on the video, so there's not much sense in analyzing it to death.
