My bike (not a Cannondale) developed a squeak similar to yours. I took it to a bike shop where a mechanic cleaned and regreased the bottom bracket, tightened the pedals, plus a few other things. The squeak was not audible on the bike stand, but persisted when I rode the bike.

Eventually the mechanic noticed that the rear mudguard was slightly out of alignment, and was grazing the wheel!
