I saw something simlar back in the 60's and the shifter was a slap shifter that was intended to give the child a car like shifter. The back in of the bike where the loop is was intended for kids to hold on with their hands while you dragged them on roller skates. I don't think that seat is original though, it looks modified to handle that odd seat. But that's all I remember.
