My tool kit (which I carry on all bikes whether it's road, off-road, touring or commuting) consists of

Tube to match tire size
Rima Tiptop patch kit with approximately 10 patches (I've used that many on rides before )
Var Tire lever
Cool Tool (much better than current multitools )
Sram power link in 8 and 9 speed (usually 2 of each)
A multisize spoke wrench
A Leatherman Squirt S4 (scissors)
A Leatherman Squirt P4 (pliers)
Extra 10-32 allen screws
Some duct tape
Zip ties
shock pump
A 16 penny nail (with a rock it works well as a punch for tightening loose bottom brackets)
A hypercracker tool for removing the cassette lock ring

When I tour, I also carry spare spokes for the freewheel side.

I have a first aid kit that I carry if I'm going a long way away from home but otherwise, I just use common sense and what is at hand for injuries.
