http://www.athletescare.com/. I saw Dr. Woollam, as did Cavernmech and Joebent. Seemed like a nice/knowledgeable guy, plus when I made the appointment he was on vacation doing bicycle touring in Italy, so as a cyclist he's probably a good person to see.
