My foot slipped off the pedal while I was torquing the handle bars. My weight shifted forward, and I somehow went tumbling over the bars.

It was a completely unexceptional crash -- I've been racing cx for five years and have suffered far worse crashes. As soon as I stood up I noticed that my left shoulder was completely f--ed up. A trip to the ER and two x-rays confirmed what the doctor already knew -- fractured clavicle. Luckily, it was not serious enough to require surgery. Never the less, my arm is stuck in a sling for the next five to six weeks.
