I have long held that Baltimore pre Bike Plan was a utopia for the VC ideal, lots of narrow lanes and low speed limit (30-35mph) roads and no bike infrastructure. What we had to show for that was a very low mode share and lots of car/bike crashes at 2-3 times the rate of NYC.
I will agree that I think treatment at intersections is critical for inducing a more vehicular attitude among cyclists.
I will note that personally I like pocket lanes over Portland's bike boxes but just about anything can work with enough education and enforcement, even VC.
