Front shifter issue

A friend asked me to look at his Santana Sovereign SE to see if I could find the problem with his front shifting. I went through the usual drill, setting the end stops correctly, aligning the cage, checking the cable tension. But nothing seemed to fix his problem of shifting from the big ring to the granny with 1 click. It seemed to me that the travel was just way too much and I wondered if there was a problem with the levers (maybe they were double levers by mistake?). The derailleur was an Ultegra 6503 and I believe the lever was a 105. Neither has triple indicated on it, although I know that the 6503 is a triple front. Not sure about the lever though. Usually I can make any drive train work given enough time, but I was hard pressed to see how to fix this. I wonder if it's a chain line issue? it seemed worse on the 2 big cogs. I admit that I didn't check to see where the chain was on the middle-middle combo. Any ideas?
