My personal favourites have evil nasty wheel crushing potholes in them or are half covered with sewer grates and manhole covers.

There are a few bike lanes here in Savannah, that literally have trees growing in them.
And it's not any better for car drivers, I know of at least one location where a tree's trunk is taking up nearly half of the lane (luckily, they did not decide to put a cycle lane on that street).

There is a bicycle lane on one of my routes to work that directs cyclists to the right of a right turn lane.

It's typical -- put together B.S. and call it a bike lane, and when the nonsense is brought to official attention, they can say, "Hey, we provided it -- what do you want, a whole lane to yourselves?" It's a demonstration of how low we are on the food chain.
