Bicycle Mechanics - Pedal extensions

I ride a Bianchi SanRemo with frogs. It feels like my feet are to close together when they are clipped into the pedals. When I sit in a chair and place my feet at a comfortable distance apart (with my feet directly under my hips) the distance is farther apart than when I am on the bike.
I was thinking - is the pedal spread the same for all bikes, cranks and pedals?
What is the distance between the pedals?
Can it be changed?
Is it possible to buy “pedal extensions” that would move the pedal away from the crank about ˝ inch?

What you are referring to is called the Q-factor - at least that's what I believed it is, but Sheldon Brown's site gives a different definition for Q-factor. The Rivendell site (http://www.rivendellbicycles.com/general/Q-Factor/) seems to agree with my interpretation. I'm not sure that how far apart your feet are when sitting should be an indication of where they should be on the bike. You should be comfortable though.

I took a look at the third hand link. You might want to note that those adapters fit a 9/16 in pedal. Almost all pedals these days are 15 mm.

I think the threads are all 9/16x20 and just the wrench-flats are 15mm.

As far as size, I was going to ask my LBS what the thread size is.
