It looks like 9, 10, and 5 just completed their exchanges in a loose stack and 15 tried to go between the 9 and 5 riders (maybe not realizing 5 was coming down) coming off the track and ran right up the back.
In the sprinter lane picture you can't tell who has the right to it without knowing who was there first.

I was at the race with the sprinting lane picture(2005 world championships). The man from france had the lane. The guy from Australia came down on him.
The frenchy got screwed big time!
As far as being in Manchester over the weekend. That was my twin brother that I don't have!
In the madison picture it looks like the '15' team just ****ed up!
This guy got back on and rejoined the race the following lap in the madison, very impressive.
The Cuban girl riding this wasn't so lucky, last corner on the women's scratch race, about 6 or 7 riders came down, not good.
