Bicycle Mechanics - Triple adjustment headache...Ideas?

I have a triple, and I can't seem to get the front derailleur to work the way I like. Here's the setup:

105 9sp brifters
Ultegra triple FD
FSA Triple cranks 28-39-50
XT RD
11-32 cassette

I have managed to get the small chanring and the middle chainring to work fine, but I can only use about 3 of the 9 cogs without major chain-rub in the large chainring. I'd like to expand that out to about 5, but no matter what I do it won't work.

I've stopped putzing with it since I rarely use the big ring (due to the problem) anyway, and I can use the full spread of the middle ring with the trim adjustment.

Does anyone have any suggestions for me to try?

My first reaction was to say most of the standard stuff, but before I got there, I thought it might be helpful to know a bit more.

Has it ever worked the way you like and degraded over time or has it always been this way?
Are you having problems at the upper or lower gears (small cogs or big cogs)?
Do you have any problems in the granny?
What do you include in "no matter what I do"?

I have a guess at the answers, but I would hate to tell you again what you have already tried thus the last question...

It's really hard to diagnose the problem without having my hands on it.
It is possible to shift the chainline outward with a bottom bracket spacer placed between the bottom bracket and the right side of the frame. These are available in 1, 1.5, and 2mm thicknesses. This is fairly easy to try but will require re-adjusting the derailleur. When I changed the chainrings on my wife's Ultegra triple to 49-39-28 it was necessary to use a 1.5 spacer to get the chainline right.

Do you have the fd cable run correctly? Look very carefully at the Shimano installation sheet. The cable must run to the LEFT the little tab adjacent to the cable anchor bolt or you will get the problem you describe.

Never worked correctly
Problem is as I move up to larger cogs
No problems in granny, but I usually only use the 3 largest cogs in granny anyway
No matter what I do: Adjust limit screws, cable tension, double check for FD alignment & height, Make sure Cranks are at proper torque.

I will check the cable routing when I get home.
