If I start to pedal hard in gear 4 sometimes it'll make a clink/clunk sound and be fine.. when I shift is it not fully shifting, or what?

Your cassette may be worn and the chain may be slipping on the cog, especially if you use "gear 4" often.

I solved that same problem by just getting rid of gear 4. Now gear 3 and gear 5 get more of a workout.

Your derailleur probably needs an adjustment. If you want to do it yourself, go to the Park Tools website, they have some detailed instructions.

Could be a worn cassette, also could be a bad link in your chain. Backpedal the chain through while you're off the bike and check for any kinks.
