Does one mount the take-off adapters over the bar tape or mount first and tape over them? I can see doing it either way.

Take-Off brackets are mounted so their tabs are caught and clamped under the brake lever mounting bands. That's what hold them in place. You mount them first and bar tape over them as you tape over the brake lever band.
