I cannot get my Campy 10sp Bar End Shifter to index properly. The upper & lower limits are set correctly. When it's indexed correctly for the large spockets, it shifts straight from the 13 to 11 when down shifting, missing the 12 cog despite there being 1 click left on the shifter. If it's indexed so it shifts correctly on the small cogs, then when I get near the 19, then it starts over shifting upwards missing the 21 & jumping straight on the 23. New cables, chain & sprockets that have been used together. Happens in the 39 or the 53.

Check the position of the cable where it is clamped to the rear deraileur. There is a tiny little groove that the cable must sit in or it becomes impossible to get proper shifting. The Campy rear derailleur manual (a PDF version is available on www.campagnolo.com in case you've lost the original paper copy) has a little diagram showing what the proper routing is.

You shouldn't be setting the adjustment for indexing at one or the other end of the cogs. Rather, you're supposed to adjust the idler wheels to properly line up on the 4th cog from the high (smallest).
