I have a Campy Record carbon crankset with a double chainring, and would like to convert to a triple on occasion for some steep rides.
Can I keep the spider and just bolt on a triple, or does this require a full change out of crankset, bb, front and rear derailer?

The spider on a double doesn't have any mounting points required to mount a small (74 mm BCD) granny ring so you will, at a minimum, need a new crank and bottom bracket. You will probably also need a new front derailleur but that's not certain. Your double rear derailleur will work if you avoid the smallest two or three rear cogs while using the granny ring. You are fortunate that Campy Ergo shifters are double/triple compatible.

You would have to replace the crankset, BB, and both deraileurs. You may want to consider the new Campy compacts, there is a 50/34 and soon to be 50/35 and 48/34 with corresponding front derailleurs.

No. Your Record spider has a 135 BCD Bolt Circle Diameter. The smallest chainring that will fit is a 39t. The compacts are built with smaller BCD's.
Don't know which cassette you have but you can run a 13-29.
