Koga built it with Dura Ace somewhere early eighties. What year exactly is something I still have to figure out. It is pretty much original. I took the bell off.

Size: You have a good eye ;) There is a sticker on the frame that says 58cm, but it defo isn't 58. 56 c-t sounds more plausible.

Here I have two more pics of particular frame features. The 1st is showing the cut out 'M' of Miyata the 2nd shows one of the Shimano dropouts:
