How are riders setting up in terms of the relationship of the knee to the pedal spindle? I'm not looking for a debate, but rather considered opinions and knowledge based on experience.

There is no relation when talking about Tri/TT setups, which typically have effective seat angle of 78+*.

In that case, how does one go about determining the fore/aft saddle position?

Here are just a few articles on the subject...

The first two articles go together, and I prefer them, even though they're older, to the last article. They makes more sense to me and seem simpler. The third article is a report of some scientific studies on seat angle.

Basically, it's an individual thing and something that you need to experiment with to find out for yourself. However, I will caution you that, while experimenting, give yourself a reasonable time to adapt to the new position before writing it off.

Angles that you want to stay aware of are knee bend as this sets your saddle height and the angle of your upper arm to torso as this provides skeletal support (ie. comfort) while in the aerobars.

You'll find that as you move the saddle forward you'll need to raise the saddle a bit and you'll be able to move the bars down and away from you (ie. get lower in front). The real key is finding the right balance between aerodynamics, power and comfort.
