I can't keep the brakes adjusted on my 1995, F600 Cannondale. After 2 or 3 rides the levers practically touch the handlebars. I can take up slack in the cable and torque down the clamp as much as I dare, use the barrel adjusters to get my levers just right, than after a couple of more rides, same problem. What am I doing wrong? Also, what do you suggest as a upgrade for this bike since the brakes were never too good to start with?

This is definitely not normal- there must be either: something slipping(like a cable clamp), split cable housing that compresses, or unusual brake pad wear. Hope this helps..
