Bicycle Mechanics - Clunky rear shifts....how do I fix?

My wife is riding a 2001 Trek X01 cross bike. It has RSX shifters and rear der. The shifts are very hard and clunky. Not smooth at all. What should I be doing to fix this? What should I look for?
Sorry bud, not enough info to really help, but look here:

Is the shifting sticking in both directions or just one? Is it skipping cogs and if so is that in one direction or both? Final question, if it is skipping cogs in one direction is it sticking going in the other direction?
If it is skipping one way and sticking in the other then you need to adjust the barrel adjuster on the back of the derailleur.
If none of this applies then please provide more information and we will try again.

I am new to this diagnostic thing and bike mechanics. I am not sure what all info you need.
I will ride it tonight and try to get a more detailed report posted up.

Not wishing to prejudge but, is your wife easing off on the pedals when shifting? Just a thought.

Current (even RSX :)) index shifting is pretty tolerant of shifting under load so unless his wife is really standing with her full weight on the pedals while shifting, it should be reasonably quiet and smooth. I expect it's either a cable adjustment or dirty cable problem.

I rode the bike last night and was pushing pretty hard going thru the gears and they shifted really hard. I was in the saddle when accelerating. I'll ride it again tonight and see if it is both ways.

Went for a ride on it tonight. Clunky both ways. Going from the big on the rear down to the small is the roughest. Going back up is a little rough but not nearly as much. It usually skips the 2nd cog almost every time.
