Singlespeed & Fixed Gear - Chain tension...

So with the help of a generous friend, I just restored a Schwinn Voyageur 11.8 frame and converted it into a singlespeed.

Last night, after noticing some chatter on uphills, I threw my chain and ate asphalt.
I want to keep this from happening again.

I'm assuming that the chain was simply too loose for the hill I had just started up and that's where my issue stems from. Should I be looking for some chain tensioners?
If so, what kind?

Should I be tightening my chain tension every time I ride?
Should I replace my high-quality quick release skewer for a bolt-on?

Any help would be appreciated. The frame has full horizontal dropouts, but not track dropouts. The original bike also came with some small pieces which sat BEHIND the axle in the dropouts that seem to be spacers of some kind.

Check your chainline first. Make sure it is straight. Your chain, when tightened, should have ~1/2 of vertical play in it. Some like more, some like less. Kinda a preference thing if you ask me, but if it is too tight your parts will wear much faster then with some play. You chain should be re-tensioned when it needs it, which is not after every ride. Ditch the QR skewer.

Chatter sounds more like poor chainline then a loose chain. Was the axle still in the right spot after the chain dropped?

How tight are you clamping the qr down? Horizontal dropouts with QRs worked for years but they have to be clamped really tight.

The chainline looks great.
The axle didn't seem to have been moved.

The QR is TIGHT.
I am a really big guy and I'm clamping it down pretty dang hard.
