Ever have a shoe get stuck on a spud? Well I did this am. Went to put my right foot down and can't click out. Ended up having to unstrap the shoe and forcing it out. Glad I wasn't at a busy light.
That sucker was in there.

Cleat loose, or just worn out.
They are almost new.
I think my foot didn't come down properly and the front of my spd somehow locked into the toe region of the cleat. Scary.

Check the spring tension
Lube the spring and the locking mechanism
Esure that the cleat is tight

Every time this has happened to me or someone I know, one of the screws came out of a cleat so it was rotating on the bottom of the shoe.
One of the little springs was locking. Good call.

A lot of people think that SPD pedals (and other clipless types) are maintenance free. They're not. They work a lot better if you occasionally put a drop of oil on the springs and the pivot points.
