It's called a "Problem Solvers 1:2" It allows 1 lever to operate two brakes. QBP part number is BR3341 so most any LBS can get one for you. They're not cheap, regular retail is around $50.00.

You'll have to call the guy, but he's fast at sending out stuff. And MUCH cheaper than $50, I think only $10.00
