My new pair of Sidi Titans (size 45.5) feel too narrow (sides of feet are hurting), but the length size is about right (maybe a tiny bit small but nothing that is bothering)
Would moving up to a size 46 help this or should I look at a different shoe all together?

Take a look at the Sidi Mega series, the Sidi's general run narrow. The mega series they sell are wider.

When trying bike shoes, the width of the sole should be pretty close to the width of your foot at the "ball" region, i.e. where the shoe contacts the pedal. Not wider, and definitely no more than slightly narrower.

Yes I have to go with Kev... try the Mega they are made foe the wider feet.
