I was trying to tune my gears and made it worse. I've re-attached the cable, pulled it tight, but the derailleur won't move beyond the 3rd cog. I've turned the barrel adjuster counter-clock wise to no effect. Please help. I have a Shimano setup.

Disconnect the cable, and pull on it while shifting (your shifter) all the way to the smallest cog.
Look inside the shifter and make sure the cable end is seated properly in the mechanism.
Sounds like your shifter was not in this position when you started.
