Give Nelo @ Nelo's Pro Cycles a call. I was having back and neck issues and he adjusted my position/fit and solved my issues.

Wenger at Source Endurance (http://www.source-e.net). Pricey, but worth every penny.
I really have no dog in this hunt as I've never been fitted. I would look for a shop that is focused on your type of riding. For instance, if you're looking for a racing fit, then Nelo's would be a good bet. Time trial or triathlon fit, then Jack & Adams, endurance or recreational or comfort fit, maybe someone else. Just my $.02.
I'd have to say that Tom Ernst at Richardson Bike Mart in Frisco is
one of the best fitters in the area. He's a certified Serotta fit specialist and
knows a other fitting techniques.
