Nutritional/supplement experts, I need your help.
This isn’t really related to me or cycling, but I need your help. I have a STUBBORN grandmother who seems to be addicted to slim-fast and won’t eat anything else. Luckily she mixes it with whole milk, but she’s still not taking in enough nourishment. We’re looking for a true meal replacement shake that will give her a balanced diet without a lot of sodium. Anyone have any ideas?

Here is a link to some meal replacements. but just about any protein powders will work and can be considered Meal replacements.
But, and I will stress this but,
Eating these alone voids the system of much need complex carbs and fiber.
You can also get a juicer and try to get her to drink juiced vegtables and fruits and this will at least allow her to get in some fiber. Or at least take some fiber 1 mix.
It is just there is no substitute for eating whole meals.

Is there a reason she won't eat meals? My wife's grandmother (95 on Saturday) doesn't like to cook a meal, because she is alone, and she thinks it will go to waist. We used to make some meals and bought tupperware that has the dividers to keep the food separate. We would then put them in her fridge or freezer and she would just put them in the microwave. Not as good as fresh home cooked, but better than what she was eating.

I believe Hospitals and other institutions use a product called Ensure, which is available at the pharmacy and most food stores. If I'm not mistaken, it has a few more calories than Slim Fast. You might want to look at the others available as well, check the labels for nutritional information. I would of course also check with a medical doctor. Blood tests can also reveal if her nutritional needs are being met.

My grandmother stopped cooking for herself during the first stage of Alzheimer's. She couldn't remember how to operate the microwave or oven.

You need to talk to her, and determine why she is just using slim fast. She is running the risk of GI problems. The Human body is designed to use and requires a little of everything every day. You can go a few days (weeks even) Missing a few of the fat soluble vitamins every once in a while is ok, but some you NEED everyday. Fiber is definatly a daily requirement. It keeps everything moving and functioning like it should.... She wont get nearly enough in slimfast.
Does she have dentures? Is she having problems with the dentures? Are her teeth hurting her?

Ensure as mentioned above is probably about the best thing you can get in liquid meal replacement but it definitely won't serve permanently.
