42/18 free (use it for rail-trails, around town riding, etc).
45/16 fixed
My bike has a double chainring. The 42 is an old shimano 600, the 45 is a salsa. The 18 freewheel is a shimano bmx, and the 16 cog is a surly. The offset on the rings fits perfectly with the different cog positions on the suzue flip/flop hub, and I always have a perfect chainline. I use a kmc track chain.
My fave is 52x16.
Currently am running 48x17 and 48x20 and 52x20. (I own several fixies and swap them around a lot).
My current fix has a 48x17. The one that I'm building will have a 39x13 if my LBS's supplier ever gets it together and sends me the stuff. I ordered it over 3 weeks ago.

44:16 on the fixed 165mm cranks
44:18 on the road SS 170mm cranks
32:16 on the SS MTB 175mm cranks
44X16 - 44X18 in the winter (have not switched yet) - SS, not fixed.
48x18 on the road/fast-commuter, 40x17 on the mountain/lazy-commuter. For what it's worth, both of these are with basic platform pedals, not clips and straps or clipless.
52/18.
It's strange because I have two bikes with the same gearing above but one frame (Peugeot) is larger and I don't ride it anymore. My main bike, (Fuji) is my size but when I first got it, I noticed the same gearing felt harder on the smaller frame. Of course, geometry plays into all of this but it was a surprise nonetheless. I'm conditioned to this gear now. I tried out a friend's 42/16 the other day and it felt strange to spin so much.

on ym surly i have 48 16 with 170 cranks and on my gios I have a 49 17 with 165 cranks

42x16 on the Slingshot. Although with 650c wheels it's probably more like 40 or 41x16. Anyone know the correction factor?

A 42/16 with 650C wheels is about a 5.1m rollout which is like a 39/16 with 700C wheels.
47/17. I'd like to go a little lower (47/18) but the damn cog is stuck on!

I just built up a Raleigh road bike fixed at 52x17. I might take it down to a 42t chainring and then do 17t and 14t on my flip-flop. It's set up with 700c and 170mm cranks. I also have a ss mountain bike at 32x18. Even though it's atleast 5 pounds heavier I can get it rolling faster. I personally find the larger gear ration easier when you have a freewheel because you have a chance to rest once you get the rotation action. It's also harder to get the peddles in "take off" position when you have a big chainring.

42x16. I have only been running the bike for about 7 months, but here in the san francisco area, this is good for getting me up the hills. I just did 45 miles on sat. and I am finally feeling comfortable flying down the hills. I was able to slow the bike and not depend on the front brake. I haven't put in as many miles as I want to on the fixie, but I'm getting there.

I could probably get away with it. Then again riding in the city and having no health insurance makes me think twice before going brakeless.

i've run a lot of different gearing.
currently, i'm running the lighest gear i've used in a while, 45x16 and i like it. i'm riding fairly casually around town, running errands, joy riding, going to see my friends, etc. i don't need super speed on this bike, and i'm also super cautious about doing damage to my knees. even my last long-term gearing, 48x16 felt like it stressed my knees in bad ways when starting from a stand-still. (i've got a front brake to help with the other end).
