Have pain on outside of right ankle just at or below ankle bone
day or two following 200k...feels maybe like a tendonitis issue.
Suspect that shoe needs to angle a bit differently, but do not know
which way to move...pain still here after one week...did ride 50
yesterday and actually felt better after first hour...little bit less sore today.
Should I move my heel in or out?

I had a similar problem many years ago. It was the cut of the shoes - the upper edge was pressing on the tendon or ligament just below the ankle bone and causing inflammation. Tried all kinds of fiddling, but the only thing that worked in this case was a new pair of shoes.

Found out that outside ankle pain was due to work Look (keo)
cleats....walking around wore outside edge down, creating a outside
edge down pedal stroke..replaced worn cleats...now ankle pain
resolving....this is a common issue...but hard to find solution..

Don't be too eager to change what has worked in the past. Chances are that you just have an overuse injury from a sudden increase in mileage. Any change you make to relieve stress in one area is likely to increase it in another and may just shift the injury to another area.
