Is there any special method for attaching old-style brake cable clamps (Simplex clamps on a PX-10)? Do I put anything on the inside of the clamp to protect the paint beneath?

A layer of clear hockey tape or almost any other sort of adhesive tape will do the trick. If the frame is black then just a layer or two of good ol' electrica tape.
