just bought a profile design stem adapter that converts my 1" threaded stem to a 1 1/8" threadless. Can I use the Delta stem riser with this? What I'd like to do is get about 180 mm of height in total and the stem riser would do that for me with a regular threadless stem. The Delta riser says it's for Ahead headsets but does that just mean threadless? Thanks for the info!!

Yes.
That being said, 180mm is an awful lot of material above the headtube. Since you did not mention
the context in which it would be used (MTB, road, recumbent?), I guess it's OK if it works for you.

I just set up one of the police bikes with a rather extreme rise; the guy was having a lot of difficulty with an old neck/shoulder issue.
I would not recommend thrashing a bike so equipped through the woods, but for just cruising around it would be fine.

Any reason that you didn't simply replace the stem with a Nitto Technomic?
