Comfort in my crotch area is important to me, but I'm not one to overlook aesthetics. It is obviously a gorgeous saddle, but will my nuts be as pleased as my eyes?

does the straight 44mm rail clamp thomson elite work well with this saddle or would you recommend a setback? (as far as saddle position is concerned)

It all depends on how your body fits the frame of your bicycle. Either one could be used to correctly position your leg over the pedal.

i've been through half a dozen saddles and always end up back on the regal. getting it just the right level is absolutely key, but then, it's like a sofa. many pros still ride regals.
