Not an LEO.. but... the term "separate piece of paper" comes to mind. Also, the phrase "accurate to the best of my knowledge and belief" might be on the applications...

OK, here's my guess at the OP:
"I was filling out an application for the local PD, and one question was 'Have you, any family member, or any friend ever use a controlled substance (illegal drug)? Itemize each incident. This does not necessarily disqualify you from consideration.' They only include one line for the answer. Can I pretend that I never witnessed any family or friends using drugs?"
