I know this is about a 2week post, it just took me a while to conjure what a starnut was.
I agree that I haven't seen one fail, either. They were a pain for me, though. One star nut was tightened a bit every week until I had to put it out of its misery when the threads of the star nut got stripped out. It did last a year and a half, though. Then I used the freestyle star nut because the bolt is obviously bigger also I can use a socket or an allen key this time. The second star nut only needed to be tighten once a month and once in a while a good hammering down with a 3/8 extension to set it back in its place. That one lasted about 4 years until it started losing grip of the fork tube when one of the tabs got bent up.
Currently and for the last 5 years I have been using a product called "Azonic aheadlock" and haven't had the need to tighten it down, not even once since I have installed it.

I had one crap out on me. They tend to pull up the inside of the steer tube over time. This can be prevented by installing it as normal and take it to a muffler shop and have them tack weld a few of the tabs to the inside of the steer tube. Works great and will never fail on you.

come to think of it i can remember one slipping up to the top of the steerer on me, but i just pushed it back in and it was fine.

Here's some knowledge from the old school: they are useless once you tighten the stem. You can safely remove the top cap after you tighten the stem. If you use something like the Azonic Headlock you can save even more weight by removing the whole assembly. Yes they can fail during installation. Tap it out the bottom, tap a new one in, and be done with it.
