I have a Santana tandem with a shimano derailleur and ultrgra shifter. When I am on the large sprocket and want to go to the middle it will fall to the small and I will have to come back up to the middle. What adjustment would take care of this problem? Could there be a problem in the shifter?

You may find that the "H" screw needs backing off a bit. It may change to the large chain-ring but because this requires too much tension on the cable, when moved to the middle it will spring back further than the middle and go to the small chain-ring.
If this doesn't work then clean and lube the FD, making sure it moves smoothly over its whole range of travel.

I'm going to assume you're talking about the front chainrings about this problem.
