It's a 1" Cane Creek threadless headest, which is about 4 or 5 years old
I'm quite sure it's the headset making the click, because the same thing has happened with the same headset on 2 different frames, 3 different forks, and a few different stems and bars.
It sometimes clicks just once, under heavy braking or going over sharp bumps, like speed humps.
I'm positive everything is adjusted properly
The cups are VERY tight in the frame.
