Mountain Biking - Bolt-on disc brake adapter

I have a mountain bike, which right now is designed for only V-brakes, and I want to get rid of them and install the Juicy Ultimate disc brakes. My question is, since the bike was designed only for V-brakes, what is the best bolt-on disc brake adapter for V-brake bikes? I already know that I will have to get a new fork (for the front brake) and new wheels, but the rear wheel is the one of most concern since a bolt-on adapter is the only way I would be able to have rear disc brakes on the bike.
Brake Therapy is probably the best but also the most pricey.

Thanks for the info. I have decided that I will be using the brake therapy hub adapter.
