Bicycle Mechanics - RD set screw adjustment?

I've never had problems with an Ultegra rear derailleur before, but today it was having a really hard time going from the third smallest cog to the second smallest in back; on almost every attempt, it failed and made this awful ratchet for at least 4 or 5 spins before I "upshifted" to stop the noise. Is this an issue with the set screws or barrel adjusters, or is it something else entirely? I seem to only get this problem on that specific gear and only when riding, not on the stand.
The upshifts are fine, but the other downshifts are a little clunky as well.

Sounds like a little cable service is in order.
Start with doing a barrel shifter adjustment. If that doesn't work it could be that your cable or cassette might need replacing.

If the inner cable is more than 1 year old, replace it. When the cable has frayed up in the lever and is almost broken, you get these sort of shifting issues. Inner cables are $5 at PBS for a pair....replace them....don't wait for to break during a ride like I do

I'd add that you should see if you might have an issue with your derailleur-hanger might not be properly aligned. If you don't want to spring around $50 for the proper tool to do the job - it's a very simple way to check and, if needed, restore it to it's proper form. Any shop can do this.
As for the cable replacement, you should also install new cable-housing in the bargain. Use the old ones to measure the correct length. When the housing get grubby inside, that can adversely effect shifting.
