Can anyone identify this freewheel? I'm actually more interested in what tool I need to remove it and reinstall on a different wheel.

It's your standard BMX/single-speed freewheel.
One of these will remove it:

Looks like it needs a Park FR 6 (4 notch).

Thx. Looks like the 4-notch tool will work on my Shimano SF-MX30 freewheel as well, although the Shimano looks a bit different. That would be a bonus.

4 notch is pretty standard for a BMX freewheel. I agree with you - the freewheel on your pic has recessed notches; Shimano has a raised ridge with notches cut into it, as I recall. Still the same tool, though.
