RD is Campagnolo Victory S3, 1988+ w/Syncros adjuster barrel, 100% positive. The barrel comes from the shifter set - which I believe is Campagnolo Syncros II.

Troublesome index, yes. In friction on a narrow-range cog (under 21-23t as largest, or thereabouts), possibly one of the most underrated straight-parallelogram Campagnolo RD's in existence. When adjusted right, they work beautifully.
